Hi List,
When did they first start printing the "SOMETHING WRONG" form that
is on the last couple of pages of every TM I have ever seen? ie: the one
with the picture of the girl saying "then...jot down the dope about it
on this form. carefully tear it out, fold it and drop it in the mail"
And what is the girl's name? (if she has one)
