 OH-6A's had a crew of 2, the pilot and a gunner. He operated a M60, in this incident, a stray bullet that was not linked fell out the door and hit the tail rotor.........which basically they took themselves out.

Also on another note, some carried the 7.62 GE mini-gun that the pilot controlled
