Two grounds per fixture is correct. Try running a piece of #12 or 14 wire
with alligators from a known good frame ground to each mounting stud (inside
the housing). See if that fixes it. Probably you will need to remove the
whole thing from the truck, and grind/sand/polish down to bare metal and put
star washers under the studs.
