I would suggest that the end float of the diff pinion drive may be out of
tolerance. You should, perhaps, check the shims on the inner pinion
bearing as a first step. This condition may arise from excessive use of
engine braking when loaded, over a fairly long period.
Finally, if the noise remains fairly constant and consistent it is, IMHO,
not likely to be warning of an imminent catastrophic failure - but it is
always dangerous to say such things from a distance.
